Getting your EAP form to Student Central

You or a 3rd party can get your form to Student Central in person, by fax, or by mail.

Once completed the student can elect to have it kept for courier mail-out on July 18th, or returned by regular mail to the student's address or if they are there in person it can returned directly to them. 3rd parties with authorization (student's separate signed and dated written authorization to the 3rd party to access the student's account for the purposes of the scholarship plan form) can have the form held for the courier mail-out on July 18th, or have it completed and returned to the 3rd party. Without authorization, the form can be completed, but it will be retained and returned by regular mail to the student's address regardless of date.

A one-time courier mail-out of these forms will be sent to the respective companies.  The deadline to submit these forms for the courier shipment is Friday July 18th, 2014. After the mail-out, copies of the forms will not be kept in Student Central. Student Central does not fax, email or mail individual plan forms to the plan administrator, nor does it fax or email to the plan holder, regardless of the date the form is received or the deadline of the company.

Student Central strongly encourages students to bring these forms in themselves prior to the courier mail-out, to ensure the fastest service possible.
