Getting your EAP form to Student Central

If you are unable to use your Confirmation of Enrolment, you or a 3rd party can submit your form to Student Central in person, by fax, or by mail.

Third parties will need authorization (student's separate signed and dated written authorization to the 3rd party to access the student's account for the purposes of the scholarship plan form) to have it completed and returned to the 3rd party. Without authorization, the form can be completed, but it will be retained and returned by regular mail to the student's address.

A one-time courier mail-out of these forms will be sent to the respective companies.  The deadline to submit these forms for the courier shipment is Friday July 14th, 2017. After the mail-out, copies of the forms will not be kept in Student Central. Student Central does not fax, email or mail individual plan forms to the plan administrator, nor does it fax or email to the plan holder, regardless of the date the form is received or the deadline of the company.

Student Central strongly encourages students to use their Confirmation of Enrolment to ensure the most efficient and fastest service possible.
